Memorial Middle School

Students interested in participating in athletics, must complete the following requirements. This includes all students who are enrolled in the athletics class 

1) Registration

All students: Register online at Rank One Sport to complete the essential forms before the first day of school. Without these forms completed on Rank One, students will not be allowed to participate.

2) Physical

Incoming 8th Graders who had a physical which was completed and handed to their respective coach last year will only have to complete the history part of the physical form which needs to be done through Rank One. These students will have a waiver and will not need to take a physical for this school year. All new to the district student athletes will be required to turn in a completed physical form to their coach along with their student id number.

Incoming 7th Graders will need to take a physical and turn in a hard copy to the front office with their student id number on it. Click on the link below to get the physical form. If you are unable to print the form, Memorial’s front office will have copies.

Incoming 6th Graders are not required to take a physical.
